Daily screening questionnaire

As described in A Guide to Reopening Our School, ECFS requires a daily screening for the signs and symptoms of COVID-19 and history of exposure to potential or confirmed cases of COVID-19. Students’ parents/guardians must complete a daily pre-arrival questionnaire administered through the Magnus Health app before their children depart for School.

The questionnaire will determine whether the student has:

  1. tested positive through a diagnostic test for COVID-19 in the past 10 days;

  2. to the best of their knowledge, been in close or proximate contact in the past 10 days with anyone who has tested positive through a diagnostic test for COVID-19;

  3. experienced any symptoms of COVID-19, including a temperature of greater than 100oF, in the past 10 days; and/or

At the completion of the questionnaire each morning, you will receive a message with the student’s attendance instruction for that day. If you have more than one student, you will need to do a screening for each student every morning.

Set Up Instructions

If you do not already have the Magnus app on your phone, please visit the Apple Store or the Google Play store to download the “Magnus Mobile V2” application. We’ve included an instruction sheet here for you to review and set up the app on your phone. If you already have the app on your phone, please check the app store to ensure it is updated to the latest version.

  1. Log into Magnus Health on your computer.

  2. Hover over your name within Magnus (top left of screen).

  3. Choose "Change Credentials".

  4. Create a username and password that only you know and will remember. This username/password combination will be used as your mobile app login only.

To access the questionnaire:

  1. Log in to the app using your newly created username and password.

  2. Click the screening button titled “COVID-19”

  3. After you complete the survey each morning, you will receive either “GO” to school or a “STOP” and stay home message for that day. If you receive a “STOP” message, please follow the directions in the app message which include notifying your divisional attendance office of your child’s absence.

All parents must submit the screening each morning no later than 7:30 AM. Timely submission of health information is vital so that preparations can be made prior to the start of each school day. Please reserve 1 to 2 minutes per child to complete this each morning.

Face/Touch ID & Push Notifications

Based on your feedback, Magnus Health has been hard at work making improvements to the Magnus Mobile V2 Application. Earlier today, they released several updates to improve the overall user experience. To take advantage of the new features we recommend that users delete the old version of the app and reinstall the new release. Please be assured that the current version will continue to work even if the update is not installed.

You may now use Face ID / Touch ID to login to the application. Click here for instructions to enable Biometrics (Face ID / Touch ID) on Apple or Android devices with authentication enabled. For users who do not have devices with biometrics enabled, you can still use the operating system’s password keeper. Click here for Android and iPhone password keeper instructions. If you have previously added the Magnus app to the keychain for Apple iOS or Autofill Service for Androids, you will not need to do it again.

Notifying the Attendance Office

Based on your responses, you will be prompted with instructions for your child to either report to school or remain home. If instructed to remain home, you must notify your attendance office at:

Uploading your Child's Vaccination Record

On the computer:

  1. Log into your Magnus Health account.

  2. Go to the Student Health Tracker.

  3. Next to the Immunization Form, click “Edit”.

  4. It will ask you if you would like to add an additional page or form. Click “Yes, continue”.

  5. It will ask you if you have already obtained and completed the form. Click “Yes, submit”.

  6. Click “Upload” to add a copy of your student’s COVID Vaccination Record Card.

On the mobile application:

  1. Log into the Magnus Mobile V2 app and click "Student Health Tracker".

  2. Click "Immunization Form".

  3. Select “Add Files” then hit "continue".

  4. Click “Add File” to add a copy of your student’s COVID Vaccination Record Card. Select the file you just uploaded and then hit “Submit.”
